They start having sex (either as teens or adults). Whoever the bottom is (up to Writer)has some past trauma and, either due to abuse or inexperience, thinks penetrative sex is supposed to hurt like hell. Also that they aren't supposed to complain about it.

It doesn't take long before the top cops to whats going on. Hes freaked out that he's been hurting his lover, but more worried about the implications of his silence over it.
