Johnny gets hurt on the same day that he's agreed to help Daniel fix something at the dojo (maybe the fence or something labor intensive)

Johnny has tried to brush off his pain, but eventually can't deny how hurt he is.

But everyone assumes he's just trying to get out of work until his body inevitably gives out - maybe it was a head injury and he has a seizure, maybe it was an abdominal injury and he has internal bleeding etc.
